Medical Notes and Working With Your Doctor
SOAP notes (medical massage notes) can be provided to you after every session to document observations and results of treatment, which a doctor (such as a physical therapist) can assess. If you would like your SOAP notes to be sent directly to your doctor you can fill out a Release of Information (ROI) with Mazy.

What Do I Need for The First Appointment?
Once you sign up for an appointment, Mazy will email you an intake form to detail your needs and medical history. Mazy will go over the intake form and your primary goals for treatment at the first session. If you have questions at any time before or after scheduling, please call or text Mazy.

Home Business
This is a home business on the lower floor of Mazy's house. There may be people upstairs. You can expect the lower floor and massage room to be a quiet comfortable setting.

Additional Information

Patients with an upcoming surgery or who are within 6 weeks of surgery should consult their physician before scheduling a massage. Medical and pre-/post-surgical massage may be available with physician approval. Massage may not be appropriate for individuals with certain medical conditions, including contagious illnesses, until resolved. Clients must be able to safely navigate a short flight of stairs, with or without assistance. Hand railings are available.
